Post-acute networks assist health systems by altering the marketplace to ensure patients access the best providers, and that transitions between those providers are effective. Because patients can still access non-preferred providers, however, a network that excludes high-volume post-acute providers can be detrimental for care coordination initiatives. Accordingly, Advisory Board recommends the following three-tier approach for building a network. This improves quality across a given geography, while maximizing the benefits of collaboration with top-tier providers.
