Innovation Submission Form for Promoting Health Equity in Cancer Care: An Innovation Showcase
Please submit this form by Tuesday, March 29 to be considered for inclusion in the Promoting Health Equity in Cancer Care: An Innovation Showcase. Submissions should be initiatives or strategies aimed at advancing equity in cancer care (e.g., addressing food insecurity or transportation issues among your patient population, data-driven approaches to improve community outreach, and implementing targeted patient navigation).

Submissions will be selected to present at the showcase based on a variety of criteria, such as thoughtfulness, creativity, and impact. If your submission is selected, you will be invited to briefly present your initiative at the virtual showcase on Tuesday, May 10th from 1-3pm. Each organization may submit a maximum of two initiatives; please submit a separate form for each. Review our FAQ for more information about the submission process and the showcase. The submission form takes about 10 minutes to complete. 

Please note: 
  • This entry will be used to determine the presenters for the health equity in cancer care showcase.
  • Before selecting presenters, Advisory Board experts may reach out with clarifying questions or a request to speak with someone from your organization about your program/initiative. 
  • All questions are required. If you do not have information to include, please indicate that in the response field.
